This is an antique cast iron heat grate or register with working louvers and outer ring. Registers such as this are used to cover holes in walls or floors for use with forced air heating systems.

This register is in very good condition with fully functioning louvers. There is some surface rusting and wear from years of use, but that can be cleaned off.
