This is an antique "Old Bridge" Inside Return W 2 Concave Ceramic 6" x 6" x 1-1/8" Tile. Old Bridge Tile Company of Old Bridge, New Jersey. Old Bridge was in business from 1890 to 1927, and was also known as Old Bridge Enameled Brick and Tile.
Good condition: showing minor signs of wear to finish but no excessive damage to the structure. There are chips, and crazing to the glaze.
